According to available data, the facility provided 100 beds—a figure that exceeds the Floyd County average by 11 beds as of Q1 2026. The center cared for an average of 86.9 residents through the period.
The CMS assigned Chulio Hills Health and Rehab an overall rating of 1 out of 5, which falls short of the 2.7 average recorded for Georgia’s nursing homes.
By the end of the first quarter of 2026, Chulio Hills Health and Rehab had received one fine totaling $4,017 and incurred one penalty.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an organization focused on insurance education. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
